(Windows, Mac, Unix) you are using, many of our forms are in

*** If you already have the Acrobat Reader, please make sure it is version

2005 Summer Camp Confirmation Packet
Please save this PDF file to your computer (using the floppy disk icon
in the top left corner of the document). Then type in your information
from the saved document , print and sign the finalization form if using
a credit card (page 7), the second page of the health history (page 11)
and the equine and/or high ropes activity waiver/release as appropriate.
You may want to save the completed documents to your hard drive for
future reference. Do not email or fax any of the forms to camp, but
mail them at least 2 weeks prior to the start of your child's session.
Reminder, please sign the equine (horseback riding) release if your
child may want to participate in a trail ride. Without a signature,
s/he will not be permitted to ride. With permission, it will be the
child's choice if they want to during the week
